IEP Services at a Glance Worksheet
A simple way to organize and understand your child’s IEP services
When you look at your child’s IEP, you may know the basics:
“My child gets speech.”
“They have resource room.”
“They receive occupational therapy.”
But do you know exactly what the IEP says about each service?
How often is it provided?
How long is each session?
Is it individual or group?
Where does it take place?
Who provides it?
And perhaps most importantly:
What is the service intended to address?
Those details matter.
That’s why I created this FREE IEP Services at a Glance Worksheet.
It gives you one easy place to pull together the important information about your child’s services instead of searching through the IEP every time you have a question.
💙 What You'll Record
For each service, the worksheet gives you space to record:
✔️ Service
✔️ Frequency
✔️ Duration
✔️ Individual or group
✔️ Location
✔️ Provider, if identified
✔️ What the service is intended to address
The worksheet includes room to organize up to six different services.
🔎 Then Look a Little Closer
Once you've pulled the information out of the IEP, take a moment to look at the details.
Ask yourself:
✔️ Are the frequency and duration clearly listed?
✔️ Is the service individual or group?
✔️ Is the location specified?
✔️ Are start and end dates included, where applicable?
✔️ Can I connect the service to a need or goal in my child’s IEP?
✔️ Do I understand what the service is intended to accomplish?
💭 What's Written vs. What's Happening
Understanding what's written in the IEP is only the first step.
Next, think about what is actually happening during your child's school week.
You may want to ask:
How is this service currently being provided?
Who provides the service?
Where does it take place?
How is progress being measured?
Is the service happening as written?
Is it helping?
And if you're not sure about something, that's okay.
Ask.
You don't need to begin with the assumption that something is wrong. Sometimes a simple question can give you the information you need.
